Transcript & Records Request

Former TvF students can request an official transcript or other school records here. Fill out the form below and we will complete the NYC Department of Education Student Records Request Form for you and send it, with your ID, to the TvF College Office.

Who can request
If the student is 18 or older, only the student can request records. If the student is under 18, a parent or legal guardian makes the request.
What you need
The student’s date of birth and a photo or scan of a valid ID for the person signing. The 9-digit NYC student ID helps but is optional.
How long it takes
Please allow up to 10 business days. The College Office will contact you if anything else is needed.

Prefer paper? Fill out the DOE Student Records Request Form (PDF) (opens in a new tab) and email it with a copy of your ID to 30Q301_collegeoffice@groups.schools.nyc.
